Custom forms are groups of the custom fields you've created. This allows you to select a subset of fields that you may want to show for a specific registration. For example, you may have a custom field called School that you use for registration for youth soccer but not adult soccer. You would then have two forms called Soccer Youth Registration and Soccer Adult Registration. These forms could include any different fields, sometimes the same field sometimes different.
To create a form click the "+" button, enter a name and click Save new form. Then add the fields you want on the form. To remove a field from a form, press the trash icon.
Using For Registration
To use a form for registration go to the season and select which form you want to display at registration time. You can test the forms out by adding someone in Dash Console or Dash Online for that season.
There are two special forms for the Customer Profile Page (Adult and Child Profile). Any field added to these form will now show up on the customer's profile page and can be changed and saved when editing the customer's profile.
Keep in mind these forms are not associated with any type of season registration. That will need to be a separate form
